Life Insurance
Basic and voluntary employee life insurance is administered by UnumProvident.
If an employee dies while covered by the plan, the plan will pay the
benefit to their beneficiary or beneficiaries as a lump sum. Coverage
limits available are based on the salary of the employee being covered.

Accidental Death & Dismemberment Insurance
Basic and Voluntary Accidental Death and Dismemberment Insurance (AD&D)
policies are offered by UnumProvident. If an employee dies or suffers
a serious injury while covered by the plan, the plan will pay the benefit
to their beneficiary or beneficiaries as a lump sum. Coverage limits
available are based on the salary of the employee being covered. More

Long-term disability insurance provides continuing income if an employee
is unable to work for an extended period of time as a result of illness,

Employee Assistance Programs
The employee assistance program (EAP) is a short-term counseling and
consultation service designed to help employees and eligible family
members with a wide range of mental health and daily life issues and
concerns. Below you will find information about the different types
of EAP coverage offered. Each service can be offered in three, five
or eight sessions.
